COVID - 19 Vaccinations//

Looking to get vaccinated? Please follow the steps below to get signed up with us! 

Step 1: Click the PDF icons on the right to download both the Baggett Pharmacy COVID-19 Vaccination Screening Form and the Moderna EUA Fact Sheet. 

Step 2: Print the COVID-19 Vaccination Screening Form and fill out ALL  information on the form.  Please do not sign the form - we will have you sign and date the form the day you come for your scheduled appointment. 

Step 3: Bring the following to the pharmacy for us to make a copy: 

          1. Completed COVID-19 Vaccination Screening Form 

          2. Medicare Card (Red, White, and Blue Card) 

          3. Insurance Card

          4. Driver's License 

Step 4: Once we have all of your information turned in, we will then call you and schedule an appointment for you to come in for your vaccination.

V-Safe is a smartphone based tool that uses web surveys and text messages to provide personalized check-ins after you receive your COVID-19 vaccination. It can notify the CDC of any side effects you may experience.

Which vaccine do you have?

We are currently only administering the Moderna COVID-19 Vaccine. \

Do I need a second dose? 

Yes! Both vaccines require two doses. The Moderna vaccine requires a second dose 28 days after the first. Booster doses are available 6 months after your 2nd dose. Immunocompromised individuals can receive a booster dose 28 days after their second vaccination. 

Do I need to stay at the pharmacy after I get my shot? 

Yes! We ask that after you get your vaccination that you stay at least 15 minutes to make sure you don't have a severe reaction. If you have had a severe reaction to a vaccination before, we ask that you stay for 30 minutes. 

What side effects should I expect? 

The most common reaction is pain and swelling in the arm where you received your shot. You may also experience a fever, chills, tiredness, and headache. The Moderna Vaccine is still being studied and these are not all of the side effects that may occur.

How soon can I get my vaccine? 

We are currently working on administering as many doses as we can throughout the day. We are currently providing vaccinations Monday through Friday between 10 AM - 11:30 and then 2 PM - 5PM. Just call down to the pharmacy for more information and availability!
